Reserve Request Form Printable, downloadable form.
Copyright law requires a form be submitted for each request.
The ever-expanding charms of the Millennium Copyright Act do REQUIRE that each copy of a Reserve Form request be signed by the requestor. The Copyright Act means that Reserve Forms must have "blue-ink" signatures--i.e. original signature only, no faxes or email. For Interlibrary loans the Online ILL Request Form requires that you enter your UID (university ID) in the barcode area.

InterLibrary Loan Request "form":
In addition to the Copyright Act, the new requirements made by BPM 66 [UT-System & its auditors] & U.S. and Texas state privacy and computer security laws and regulations mean that the library now strongly prefers to receive ILL requests only through the online catalogue system--any other method will "bounce" and require extra time to process.
